To refer an assignment for investigation, please download the below assignment sheet. Then e-mail, fax or mail the assignment to our office.

You will receive an acknowledgement within 24 hours. You can also call in your request, Monday through Friday from 8:30am until 5:00pm, Pacific. Thank you for choosing Ludwig & Associates!
